Position Overview: The Elder Care Support (ECS) Coordinator will support identified clients to navigate access to appropriate local aged care support services. This role involves providing advocacy support to ensure clients receive a culturally safe plan assessment and helping clients understand their aged care service entitlements, including My Aged Care registration, contractual arrangements, and costs. The ECS Coordinator will also liaise with aged care Assessors to ensure Elders are well-supported throughout the assessment process.
Key Responsibilities: Support clients in identifying available services and access pathways.Brief assessors, involve in assessment interviews, collaborate on support plans, and review plans.Assist clients and families in selecting and understanding aged care service providers and contractual arrangements.Ensure clients' care and cultural needs are met, and address issues with service providers.Link clients to related services and provide information on care options and costs.Actively promote aged care services to the community and raise awareness.Identify and engage potential clients through community events and outreach programs.Engage with My Aged Care and local assessment staff.Participate in training programs to stay updated with aged care policies and practices.Maintain accurate records of client interactions and support provided.Generate regular reports on service outcomes and client satisfaction.Ensure culturally safe and trauma-informed care.Navigating complex service landscapes and advocating effectively for clients.Building and maintaining strong community and service provider relationships.Qualifications & Experience: Aboriginality.
