Katherine West Health Board (KWHB) is currently looking for a Discharge Care Coordinator to join their team for an opportunity in the Katherine West region in the NT. This position will be well suited to someone with a recognised qualification in Aboriginal Health Practitioner or Registered Nurse with no restrictions on practice. With experience in coordinating care, case conferencing and discharge planning, preferably in a remote or Indigenous health context.
This role ensures seamless coordination of care and services during the discharge process from tertiary and interstate hospitals, fostering continuity of care and optimal client outcomes.
Your responsibilities will include (but are not limited to): Communicate proactively and effectively with hospital staff including doctors, nurses, and social workers, to gather relevant patient information and discharge plans.Act as an advocate for the client's needs and preferences during the discharge planning process.Ensure travel for clients returning to their home communities, whether by air, road, or other means, is appropriate and includes necessary equipment such as wheelchairs or escorts.Arrange for necessary medical equipment and supplies to be available upon the client's return home.Refer patients to relevant support services, such as mental health services, NDIS and social care services as required.Participate and contribute to continuous quality improvement within KWHB.Is this the right role for you?
