Disability Transition Service Planning and Complex Case Manager As a dedicated Disability Transition Service Planning and Complex Case Manager, your experience in disability services, knowledge of complex case management, and person-centred planning will be invaluable.
You will work collaboratively with families, guardians, staff, and individuals with disabilities to ensure positive outcomes. Additionally, you are someone who is confident in providing practice leadership and direction to managers, and you will also direct, develop, and review operational policies, practice guidelines, and standards.
You will be responsible for (including but not limited to):
Reporting on transitional planning, complex strategic service delivery, and case management issues. Preparing reports, correspondence, briefing papers, and speech notes. Providing operational policy advice to the Director and Executive Director on complex matters related to individuals with complex needs. Leading the monitoring, investigation, and compliance functions to support the administration of the service and ensure the assessment, treatment, care, and protection of individuals to support their transition requirements and complex needs that comply with both State and Commonwealth legislative requirements. Successfully influencing and negotiating to achieve optimal outcomes for the department and for individuals with complex needs.
