About the role
Specialist Disability Housing is currently looking for casual disability support worker with minimum 2 years of experience. We are looking for compassionate, reliable, and experienced disability support workers for our clients all over the Perth Area. You can make a difference in our clients' lives by supporting them with daily living activities and domestic duties and empowering them to achieve their life goals.
Working in a team, you will facilitate the delivery of quality disability services and achieve positive outcomes with service users in a positive and proactive environment.
You will be actively supporting their individual needs and creating a safe environment.
Key Responsibilities Support participants to work towards achieving their individual goals. Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration, community access etc. Support participants in building their confidence. Liaise and collaborate with clients, their community and community & disability support agencies. Monitor and review the participant's progress through the service and against the support plan. Utilise positive behavior strategies Support clients to participate in ageappropriate activities and tasks, including employment, inhome disability support and community access. Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality. Monitor the safety and wellbeing of people with disability and report any concerns to the Manager. Ensure completion of appropriate documentation. As required, relevant information on disability support will be recorded in client notes and communication books. Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes. Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals. Liaise extensively with other staff to ensure the overall smooth and consistent running of the disability services. Report regularly to the Manager Skills & Experience Qualification in community/disability/individual support/aged care/ Mental Health and preferably with relevant experience. Experience working with clients with disability & challenging behaviours. Welldeveloped verbal and written communication skills. Current Driver's License and reliable vehicle. 100point check ID Valid visa/working rights in Australia-
