Engage with NDIS participants, their representatives and support coordinators to make an assessment of needs, identify priorities for intervention and collaborate on the intervention planDesign a plan for appropriate interventions to improve the person's quality of life, skills and capacities to meet their goalsProvide therapeutic interventions directly with the participant, related to the practitioner's scope of practice, skills and experienceEvaluate the outcomes of interventions to ensure the participant's goals are met and supports are effectively being implemented by those in the person's support networkCollaborate effectively with other allied health practitioners to maximise agreed outcomes for the participantIs this the right role for you?
Tertiary Degree qualification in Social Work with full registration with the Australian Association of Social WorkersExperience working in rural or remote settings, and/or with young people and/or adults who have a disabilityExperience developing and implementing interventions within a holistic person-centred frameworkCommitment to professional development, supervision and reflectionCurrent Driver's Licence, NT Ochre Card and Worker Screening ChecksMeet billable hours as per contracted KPI's #J-18808-Ljbffr
