Qualifications & Experience Minimum Diploma or higher obtained in Australia in health or welfare (Preferably Psychology or social work) with a minimum of 1-year full-time work experience in coordinating supports to people with disability; OR Relevant overseas qualification equivalent to Diploma or higher (Preferably Psychology or social work) with a minimum of 2-year full-time work experience; OR Minimum 3-year full-time work experience if no relevant qualification. Current First Aid Certificate Unrestricted full driver's license Cleared NDIS Worker Screening Cleared police check Purpose of Role Responsible for maintaining a positive and effective team environment and coordinating support to service users accessing the company's services. The DSO will assess the needs of clients, including community linkages, mainstream, informal, and funded support.
Key Responsibilities Assess referrals and coordinate the analysis of support needs, planning, and program development for service users. Support clients to understand and implement the funded supports in their plan and link them to community, mainstream, and other government services. Assist clients to negotiate with service providers about offerings and costs out of the NDIS plan. Ensure service agreements are completed and help clients build the ability to exercise choice and control, coordinate supports, and access the local community. Assist clients to optimize their NDIS plan, ensuring they receive the most reasonable and necessary funded supports. Coordinate and ensure the development of clients' support plans (including supervising/supporting key workers in developing Individual Plans). Coordinate rostering requirements according to individual support needs of clients and service agreements. Ensure effective communication around the company service provision with families and key stakeholders, including government agencies. Communicate with line management about program and staffing requirements. Provide after-hours on-call support to teams. Fulfill any other duties as required. #J-18808-Ljbffr
