Disability and Complex Care Practice Lead, Supports and InterventionsFull-time PermanentDetails: ACT Corrective Services (ACTCS) is seeking a highly motivated and experienced person to fill the position of Disability and Complex Care Practice Lead (HP4), Supports and Interventions (SIU), within Offender Reintegration. This position is based at the Alexander Maconochie Centre (AMC) in Hume.The successful applicant will be responsible for the provision of both organisational and individual support related to increasing the capacity of individuals with complex needs within the AMC, providing specialised clinical care coordination for complex clients across the continuum of emerging, pre-existing as well as identifying unmet disability needs across the AMC. This includes supporting detainees with complex behaviours and as required, recommending assessments and interventions to develop positive behaviour support plans.In addition, you will develop and implement a range of multidimensional treatment options that offer opportunities for the development of physical, emotional, and general wellbeing for detainees with varied and complex needs, and provide advice related to the care of, and service provision, for detainees with disabilities.Further to this, you will identify and implement actions to address systemic barriers to service delivery access for detainees with complex needs in the custodial setting, including leading care coordination meetings, contribute to higher level and strategic planning forums, and take initiative to drive systemic change in the disability and complex care space for ACTCS and the AMC.To be successful, you will possess excellent interpersonal, organisational and communication skills necessary to build rapport with a diverse range of clients and stakeholders, in addition to demonstrating strong leadership and management qualities.
