Job Title: Disability and Complex Care Coordinator (HP3)
Details: A career opportunity has arisen in ACT Corrective Services for a suitably qualified person with demonstrated clinical experience, personal integrity, self-confidence, and exceptional communication skills for a Disability and Complex Care Coordinator (HP3) within the Offender Reintegration team.
The Disability and Complex Care Coordinator provides specialised clinical care coordination for detainees with disability and complex needs. The Coordinator is responsible for ensuring individual needs are assessed, implementing clinical interventions, engaging in stakeholder engagement, and undertaking functional assessments for complex needs.
Under direction, the successful applicant will deliver a range of multidimensional treatment options for detainees with varied and complex needs. You will also provide advice related to the care of and service provision for detainees with disabilities and complex needs, including contributing to accommodation unit placements and regular care coordination.
Further, you will maintain monitoring systems to enable evaluation of the effectiveness of treatment options in accordance with leading practice guidelines and participate in the ongoing review and evaluation of work practices, including quality assurance and reporting on performance indicators, and contribute to the development of policies and procedures.
To be successful, you will have a high level of interpersonal, negotiation, and communication skills, including the ability to deal with people from a wide range of cultures and backgrounds in a correctional environment.

Eligibility/Other Requirements: Registered Nurse – Mandatory Hold registration with Australian Health Practitioners Regulation Agency [AHPRA]Applicants must have a minimum of three years paid work employment, post-qualification in a relevant role.Postgraduate qualifications in a relevant area would be highly desirable.Occupational Therapist – Mandatory Hold general registration with the Occupational Therapy Board of Australia (Australian Health Practitioners Regulation Agency [AHPRA]).Applicants must have a minimum of three years paid work employment, post-qualification, in a relevant role.Demonstrated experience and/or willingness to work with detainees is essential.A current driver's licence is essential.The successful candidate will be required to undergo a criminal record check.This position does require a Police Record Check.To be eligible for permanent or temporary employment within the ACT Public Service (ACTPS) you must be an Australian citizen, a permanent resident, or hold a valid work visa.
