Our health centers service nine communities spread over 45,000 square kilometers in the Big Rivers region.

How You'll Make a Difference Sunrise Health Service now has a rewarding opportunity for a credentialed Diabetes Educator to join their Integrated Health Team on a full-time basis in Katherine, NT. Reporting to the Integrated Health Manager, this role provides a visiting service to all Sunrise clinics and communities providing individual and group services to diabetic patients and families.
This clinical role will see you providing high-quality and individualised diabetes management/care for patients and their families suffering from, or at risk of, diabetes and associated conditions. The role is part of a team of preventative and chronic disease education staff, working with remote clinical staff.
All team members travel regularly to the nine remote clinic sites, all located in remote communities in the eastern Katherine region of the NT.
Your day-to-day duties will include but not be limited to:
Working collaboratively with all health centre and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions;Providing clinical leadership to other health professionals, including Remote Area Nurses (RANs), Aboriginal Health Practitioners, and medical and allied health professional staff in the area of diabetes management;Managing relevant Medical Benefits Scheme claims for care provided as required and applicable; andPromoting a multidisciplinary team approach to decision-making and care provided.What You'll Bring to the Team This is a very busy and demanding role, but one that gives you a rewarding opportunity to make a tangible difference in the lives of patients and their families in the improvement of their long-term health management.
To be considered for this role, you must be a qualified health professional who either holds or is eligible for registration with AHPRA.
You must also hold existing accreditation as a Diabetes Educator and demonstrated professional experience in diabetes and chronic disease management.
This role would be well-suited to someone who is very strong in their clinical service delivery; has well-developed problem-solving skills; and demonstrated knowledge of comprehensive Primary Health Care Principles, including social determinants.
Importantly, you will be comfortable living and working in a remote location and have the ability to practice in a sensitive and culturally safe manner in an Aboriginal community setting.
Please note: A current open Driver's License is a requirement for this position.
Confirmation of employment is dependent on the outcome of a Criminal History Check and successful application for an Ochre Card.

About the Big Rivers Region The Big Rivers region (a 3 hour drive from Darwin) begins south of Kakadu National Park encompassing Pine Creek, then extends over 500km south to Kalkaringi and stretches across the width of the Territory between the Western Australia and Queensland borders.
The region is significant in scale, covering over 360,000 square kilometres and accounting for an area greater than that covered by Germany.
The region consists of a number of varied natural environments including vast rivers, tropical forests, rocky escarpments and savannah plains.
