About the role A rewarding opportunity for a passionate Dementia professional to join the DementiaCentre Team at HammondCare on a Permanent Full-Time basis at our Horsley site.
The role focuses on improving the lives of individuals with dementia across HammondCare services by building staff capacity to deliver best practice care.
The Dementia Centre Advisor will support staff at all levels in implementing HammondCare's Model of Care and Case Management framework.
As a Dementia Centre Advisor, you will engage in projects and contribute to knowledge generation on best practice dementia care.
You are capable of working both autonomously and as part of a team.

CPAT, BSPs etc.
)Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvementsProvide support through mentoring and modelling in the SDCP onsite (where applicable) and be active part of CRT and CAC.
This support can extend to other HC SDCPs as neededAccess current evidence-based guidance and knowledge, ensure it is aligned with the Dementia Centre and utilise to inform all activities in the roleUtilise agreed HammondCare and the Dementia Centre processes to communicate, monitor and manage work delegations and projects including evaluation and reportingParticipate, contribute and learn as part of the Dementia Centre team in team meetings, professional supervision and team activitiesWork on other projects and assignments as required and in line with areas of skill, expertise and knowledgeWhat do we need from you? A minimum of 3 years' experience in the Australian Aged Care Industry AND able to demonstrate currency in your field of expertiseRegistered Nurse or Allied Health qualification, with further education in Certificate IV in Training and Assessment (TAE40116 or TAE40110), including TAEASS502 and TAELLN411 or higher, is highly desirableDemonstrated skills in teaching, mentoring and coaching othersKnowledge and skills in relation to adult learners and development of learning resourcesHigh level of negotiation, communication and relationship building skillsAbility to prioritise multiple tasks and meet deadlinesProblem solving and decision-making skills relevant to the aged and dementia care contextStrong interpersonal and communications skills both written and verbalA current drivers licence and safe driving recordWillingness to travel between HammondCare's sites and services and across AustraliaStrong administrative and computer skills using Microsoft Office products such as Outlook, Word, Excel, PowerPoint and TeamsAbility to work independently and as part of a teamFor all questions about the role, email: ****** All short-listed applicants will be asked to consent to a criminal record check paid for by HammondCare.
