About The Role
This particular team that is hiring is the heart of the business and has a genuine interest in enabling its customers to live better lives at home. This is an exciting opportunity for a young leader to manage a team of up to 20 people, who are responsible for a portfolio of ~200 clients each. You will ultimately have team leadership experience, whereby you've been responsible for leading and managing a team of phone-based customer service representatives.
Job Description Facilitate care plans and monitor budgets while providing case management for clients.Conduct regular check-ins to assess client goals and make necessary adjustments to plans.Approach sensitive topics, such as changes in care needs or funding, with compassion and a solutions-oriented mindset.Escalate concerns related to health or wellbeing through established clinical pathways.Oversee administrative tasks to ensure compliance with Aged Care Quality Standards.Build positive relationships with clients, their families, and service providers.Your Key Skills You are a great listener.You can ask good questions.You have leadership potential.You value long-term relationships.You are good at taking on feedback.You are competitive (in a friendly way).You enjoy taking great notes and learning.Proficient computer skills with knowledge of Microsoft 365.You're known to have an empathetic leadership style; you can lift others up!If you'd like to know more information, please confidentially contact me to learn more about this unique opportunity, and please apply at the appropriate links provided.
