About the role As our new Customer Coordinator will you enjoy providing exceptional customer service and effective and efficient care solutions to meet our customer's needs in the Bundaberg region. 
Day to day you will maintain a designated case load of mixed funded packages, including Home Care Package customers. In meeting with customers in their home, you will gain an understanding of their needs and subsequently involve the relevant stakeholders in the assessment, planning, implementation, and review process to ensure that our customers achieve the goals and outcomes as identified through their personalised support plan. 
In this full-time permanent position, Monday to Friday (8.30am – 4.36pm), you will join a dedicated and supportive team of Customer Coordinators contributing to Suncare's growth and excellence in the age care industry. You will be servicing the Bundaberg region. There is the opportunity for hybrid work arrangements that include working at our Bundaberg office and from home.
About You In addition to your customer service ethos, you will possess a passion and positive attitude for working in community aged care. You will bring to our team:
A qualification in social, human services, or community services, and/or experience in individualised service planning, referral, and case management  Excellent communication and interpersonal skills including conflict resolution and negotiation Effective organisational skills and an ability to work with minimal direction and supervision  A proven ability to achieve effective and efficient customer outcomes, including experience working with a wide range of services and groups Knowledge of community sector support services and preferably experience working with Home Care Packages  What we offer At Suncare our people really are our greatest asset.
