The purpose of the role is to provide diabetes education, assessment, and intervention for patients and their families within the primary and community health service of AWH. To provide expert level consultation to, and integrated care planning with, internal and external service providers. To also participate in diabetes education service planning and development for continual service improvement that is aligned with local population health needs and service context. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity.Relevant undergraduate qualification in related discipline with Postgraduate qualification in Diabetes Education/ManagementMember of the Australian Diabetes Education AssociationCredentialled Diabetes EducatorCurrent valid Driver's LicenceAdvanced current knowledge and skills in the clinical management of people with diabetesKnowledge and skills in contemporary service models and self-management approaches to chronic illness careAbility to plan, implement, and evaluate diabetes education and management programsExcellent interpersonal, communication (written and verbal) and presentation skills, with ability to work as part of an interdisciplinary team.Employment Type: This is a fixed-term part-time (16 hours per week) opportunity for a period of 12 months.For further details, please refer to the Position Description or alternatively contact Allison Cummins at for a confidential discussion.About Albury Wodonga Health (AWH)AWH serves as the largest regional healthcare provider between Sydney and Melbourne, situated in the border cities of Albury and Wodonga.
