Collaborate with general practices and stakeholders to embed the Life! Program, focusing on early intervention and improved health outcomes for those at high risk of type 2 diabetes and cardiovascular disease. If you're a proactive professional with project management skills and a commitment to positive change in primary care, we'd love to hear from you.In this role, you will:Develop and implement a data-driven model for diabetes prevention in primary care.Lead project planning, reporting, and toolkit development.Deliver webinars and training to support engagement strategies.Build partnerships with general practices to increase Life!
