The opportunity: Join us as the Coordinator – Diabetes Program and make a meaningful impact on diabetes prevention across Victoria.
Collaborate with general practices and stakeholders to embed the Life!
Program, focusing on early intervention and improved health outcomes for those at high risk of type 2 diabetes and cardiovascular disease.

In this role, you will:
Develop and implement a data-driven model for diabetes prevention in primary care.Lead project planning, reporting, and toolkit development.Deliver webinars and training to support engagement strategies.Build partnerships with general practices to increase Life!
Program referrals.Support healthcare improvements in rural and remote areas.Oversee project evaluation for effective strategy execution.About you: To be successful in this role you will have:
Relevant tertiary qualifications and/or approximately 5 years of experience.Experience in project management and quality improvement activities.An understanding of the Australian primary health systems and the barriers to achieving positive health outcomes.High level of interpersonal and compassionate communication skills and experience in change management.Professional acumen for community engagement, business innovation and systems integration.Tertiary qualification in Public Health, Health Management, Nursing or related sector.Primary healthcare or health promotion sector experience.Data analysis experience.How to apply: Please submit a resume and cover letter detailing your suitability for the role.
