We also recruit, train and support workers, carers, and volunteers to work in our communities.About the roleCoordination of Support is a funded support under the NDIS.
This support helps people with disability accessing the NDIS (participants), to choose supports for themselves and to work towards their goals.We walk alongside people with disability and their support networks, whilst empowering them to increase independence, build relationships and work meaningfully toward their goals.We support participants to implement their plans, understanding the NDIS, and promoting choice and control.Benefits and perks6 weeks annual leave plus leave loading - Pro rataSalary packaging – including remote area packingSupport and Training as required/neededCollaborative and supportive team environment with a great office cultureSkills and experienceResponsibilitiesSupport implementation of all supports in a participant's plan, including informal, mainstream, and community, as well as funded supportsAdhere to a two-week cycle, ensuring each participant's case is worked on once a fortnight, and contact is made with participants at least monthlyAdhere to billable hours set out by the ManagerKeep accurate, detailed, and complete records as case notes, to be completed on the same business dayDemonstrated experience in utilising a range of supports and services to help people work toward their goalsAbility to form collaborative partnerships with participantsDevelop and maintain professional relationships with service providers and key stakeholdersCoordinate assessments, reports, and service proposalsProvide the NDIA with reports on outcomes and success indicators within the agreed reporting frequencyNegotiating on behalf of the participant around pricing and service agreements, monitoring, and reporting on service deliveryPromote empowerment, choice, and controlPromote community inclusion of people with disabilities and their families consistent with individual plansDemonstrated skills in establishing empowering and supportive partnerships with participants with complex needsSolid understanding of strengths-based, person-centred practiceUnderstanding of restrictive practices and the guiding principlesComputer literacy in the full suite of Microsoft Office as well as competent data entry skillsA sound understanding of relevant legislation, NDIS procedural guidelines, and the full range of community services that may be required by participantsKnowledge of local service providersA commitment to work within the NDIS Policy and funding frameworkDesired CriteriaCertificate IV Disability Services or equivalent (or willingness to obtain)Lived experience of disability, either directly or as a parent or family member of a person with a disabilityMust be a team playerCreating a Safe EnvironmentNT Friendship & Support ensures a safe working environment for all staff and clients by ensuring all staff have a:SAFE NT Working with Children Ochre CardNDIS Worker Screening CheckValid NT Driver's LicenceCurrent National Police Check (less than 3 months old)To apply:Please forward your resume with 2 professional references to Human Resources, Ref: COSK23, Email: ****** organisation reserves the right to move forward on interviews for candidates as deemed appropriate prior to closure.
