About the role
Coordination of Support is a funded support under the NDIS.
This support helps people with disability accessing the NDIS (participants), to choose supports for themselves and to work towards their goals.
We walk alongside people with disability and their support networks, whilst empowering them to increase independence, build relationships and work meaningfully toward their goals.
We support participants to implement their plans, understanding the NDIS, and promoting choice and control.

Responsibilities
Support implementation of all supports in a participant's plan, including informal, mainstream, and community, as well as funded supports Adhere to a two-week cycle, ensuring each participant's case is worked on once a fortnight, and contact is made with participants at least monthly Adhere to billable hours set out by the Manager Keep accurate, detailed, and complete records as case notes, to be completed on the same business day Demonstrated experience in utilising a range of supports and services to help people work toward their goals Ability to form collaborative partnerships with participants Develop and maintain professional relationships with service providers and key stakeholders Coordinate assessments, reports, and service proposals Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency Negotiating on behalf of the participant around pricing and service agreements, monitoring, and reporting on service delivery Promote empowerment, choice, and control Promote community inclusion of people with disabilities and their families consistent with individual plans Demonstrated skills in establishing empowering and supportive partnerships with participants with complex needs Solid understanding of strengths-based, person-centred practice Understanding of restrictive practices and the guiding principles Computer literacy in the full suite of Microsoft Office as well as competent data entry skills A sound understanding of relevant legislation, NDIS procedural guidelines, and the full range of community services that may be required by participants Knowledge of local service providers A commitment to work within the NDIS Policy and funding framework Desired Criteria
Certificate IV Disability Services or equivalent (or willingness to obtain) Lived experience of disability, either directly or as a parent or family member of a person with a disability Must be a team player Creating a Safe Environment
