About Us Chung Wah Community Care (CWCC) provides care and support to the aged and disabled community, particularly non-English speakers, in Perth.
We provide culturally appropriate aged care and disability services to clients from the Chinese, Vietnamese, Cambodian and other ethnic communities throughout the Perth metro. We also deliver a series of general community programs and events for Asian migrants and the community at large.

About the Role We are looking for experienced and enthusiastic Consumer Support Advisors who will play a pivotal role in delivering comprehensive support to Aged Care Consumers and Participants. Using your wide knowledge and experience, you will meticulously assess their individual needs, coordinate tailored support services, and uphold the highest standards of care aligned with our clients' individual plans.
Your responsibilities Be part of the Call Centre team, processing inquiries and providing support.Utilise scheduling techniques to manage rosters efficiently exercising judgement in planning assignments and optimising resource allocation.Processing changes and cancellations from clients and issuing assignments to cover changes while ensuring all changes are communicated to consumers and participants.Work with all stakeholders to ensure timely assignment of services to clients while maintaining transparent communication.Maintain up to date and accurate data records in the Consumer Database (Echelon) at all times.Perform comprehensive assessments and tailoring support for aged care clients and participants.Regular reviews to ensure personalised support while working closely with clients and families to deliver services aligned with individual plans.Generate up to date reports for Senior Management, for strategic decision making. We are offering 2 full time positions, fixed term for 12 months with the possibility of extension. These roles will work form our office in Northbridge.
