About the facility Our major sites include University Hospital Geelong, in Geelong's CBD, the McKeller Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region.
Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
The Connected Care Program aims to deliver better health outcomes and reduce hospitalisation rates by improved integration of care with existing GPs, providing specialist assessment, negotiated, comprehensive care plan and early escalation pathways for clients.
The service provides targeted and individualised service to consumers with complex chronic health or psychosocial needs at risk or recurrent ED presentations, admissions and readmissions.
The service is primarily delivered in the home, with telehealth, phone or in-clinic appointments at Barwon Health facilities as required.
About the role As a Connected Care Senior Clinician, your role will include:
supporting the planning, implementation and organisation of care aligned with the Connected Care model;providing clinical supervision for Connected Care clinical staff;provision of, and supporting others to provide, direct client care through a consumer centred approach;contributing to a learning environment by supporting staff and undergraduate development, wellbeing and satisfaction;leading, participating and monitoring quality improvement initiatives, in accordance with Barwon Health Policy and Strategic Plan. To be successful in the role you will have: a clinical (nursing or allied health) background with extensive clinical experiencedemonstrated ability to work effectively as a member of an interprofessional teamtraining and/or experience in provision of clinical supervisionexperience in comprehensive client assessment and client centred care planningadvanced written and verbal communication skillsproven computer literacy and ability to learn specialist programs.
