The Complex Inpatient Care Coordinator will focus on coordinating complex admissions and discharge pathways to optimise patient care, reduce length of hospital stay and facilitate seamless transitions between inpatient and community-based services.Key responsibilities include:Work as part of a multidisciplinary team to ensure safe, effective and timely discharge planning for patients with complex needs to achieve optimal patient outcomesAssess patient discharge needs and plan a response that addresses the social determinants of health, aligns with evidence-based models and supports continuity of careAct as the primary liaison between Health Service Departments and external care providers to support service system navigationMonitor and report on discharge trends, outcomes and barriers to inform service improvementsAdvocate for patient needs with internal and external stakeholders to ensure equitable access to servicesQualifications/experience required:Minimum Bachelor Qualification of Social WorkEligibility for registration with the Australian Association of Social WorkersMinimum 7 years' experience in practice of Social WorkNDIS Worker Screening CheckWorking for ERHAt ERH we offer a supportive and inclusive culture that offers opportunity for growth and development.
