* Gain Experience in senior nursing role * No shiftwork: Business hours Monday – Friday * Job Share considered An appealing fixed-term opportunity exists for a skilled and experienced Registered Nurse to gain experience as a Clinical Nurse Consultant.
If you have a passion for improving the outcomes of clients with complex and/or chronic disease, this is a great chance to expand your skills and knowledge.
The Contracts: Fixed term, Part Time, 48.0 hours per fortnight until August 2025.
Fixed term, Part Time, 64.0 hours per fortnight until June 2025.
Job share will be considered.
The Position: In this role you will facilitate service access, undertake comprehensive client assessment and coordinate the development and implementation of the multi-disciplinary goal-based care plan.
This position will ensure best practice in care coordination, service delivery and efficiency of resource usage to ensure delivery of multi-disciplinary client-centred care that promotes health independence and prevents unnecessary presentations or admission to hospital.

Key skills you need: * Current APHRA registration as a Registered Nurse with a minimum of 5 years acute nursing experience * Recent nursing experience in chronic or complex disease management with advanced clinical assessment, screening and referral ability * Sound understanding of client self-management and proven ability to provide strategies for symptom understanding and management * A high level of experience in client and carer education within the chronic disease management framework.
