About the Manager:
You'll be directly supported by the Ambulatory Care Service Manager within the John Hunter Children's Hospital (JHCH), a leader committed to fostering a collaborative and inclusive work environment. The manager provides hands-on support and guidance, ensuring you have everything you need to succeed in your role.
Our Ideal Candidate:
We are looking for a passionate Complex Care Coordinator from an Allied Health or Nursing background with strong paediatric experience. If you're compassionate, driven to make a difference, and have excellent interpersonal skills, we'd love to have you on our team.
Ideally, you:
Have a deep understanding of paediatric services across the Hunter New England Local Health District (HNELHD) Are skilled at building relationships quickly Excel in connecting families with the right clinicians, advocating for your patients, and coordinating care for children with complex and chronic health conditions. Key Responsibilities:
In this role, you will coordinate and navigate care for children and young people with chronic health needs across HNELHD, ensuring that consistent and effective care coordination practices are in place district wide. You'll be part of a broader model aimed at improving the care journey for medically complex children, helping families access the support and resources they need.
This position offers a meaningful opportunity to work closely with Hospitals across the district and patients' local services to streamline care coordination and improve outcomes for families in need. Based in Newcastle, you'll play a key role in the Hunter New England Local Health District's efforts to enhance the healthcare journey for children with chronic and complex medical conditions.
