This is an exciting time to join Sunnyfield as we embark on the expansion of our services across New South Wales.About The RoleAs a Community Support Worker based in our Singleton and Muswellbrook Hub, you will play an essential role in supporting and empowering people with disability to live a life of their choosing.
This will include working across community access supports and in our day programs.
Working in both a team environment and independently, you will provide a range of services assisting clients to live independently, be actively involved in their local community, and engaged in meaningful opportunities.Your Key ResponsibilitiesCommunity Integration: Supporting clients to live an active lifestyle and participate in their community through the delivery of a range of person-centred programs.Life Skills: Bring your supportive attitude to help build capacity for our clients in areas that they are passionate about.Personal Care: Assist and support clients with personal and hygiene care.Behaviour Plans: Adapt to clients' individual needs, working with behaviour support plans by utilising positive behaviour strategies.Family Interaction: Communication with families about clients' progress.Transport: Driving clients to and from our day programs in our Sunnyfield van.Admin Tasks: Maintenance of progress notes and record-keeping.This permanent part-time position will require you to work some afternoons and 1 weekend a month.
