They are looking a full time Community Support Worker for their centre in Brisbane, QLD.

This is for a full time role with an annual salary of $70,000 to $90,000 depending on experience plus superannuation and will be based at their Brisbane, QLD location.
Duties will include: Study the care plan and implement the support, education and training program in placeObserve, monitor and record clients' progress through case notes for every shift.Attend PBS meetings when required, participating in discussions whenever an assessment is required to review the client's community needs and resources for health, welfare, housing, training and other facilities and services.Provide support in clients' houses and provide education and care for children and disabled persons in adult service units, group housing and government institutions.Support young clients to overcome social, emotional, and financial problems.Assist in preparing reports for necessary submissions to government entities or other stakeholders for funding and resources and reports to government bodies and other agencies.Supporting clients on community outings, e.g., shopping trips, excursions, lunches, appointments, and goal-based activities.Escorting clients to medical and allied health appointments, hospital visits, and respite care.Cleaning, tidying, and changing bedding to ensure a safe, clean, and sanitized environment for our clients.Requirements Certificate in Individual Support or Certificate in Community SupportMinimum 2 years of relevant experience in a professional settingMinimum 2 years of experience dealing with the characteristics and social aspects of mental and emotional disturbances and developmental disabilityPolice ClearanceNDIS Worker Orientation CertificateNDIS Screening CheckManual Handling CertificateDriver License and own insured vehicleHave a current or be willing to obtain a First Aid (HLTAID003 or HLTAID004) and CPR (HLTAID001) certificateHave a present or be willing to obtain a Working with Children Check
