About SWAMS: The South West Aboriginal Medical Service Ltd (SWAMS) is an Aboriginal Community Controlled Health Organisation, founded on the principles of self-determination, empowerment and freedom of choice. Our mission is to provide high-quality, culturally appropriate, and holistic healthcare that builds strong and healthy Aboriginal communities. We act with integrity and are dedicated to excellence. We have over 200 staff here at SWAMS and are continuing to grow. We are actively working to "Close the Gap".
We operate out of the Greater Bunbury area, Mandurah, and have numerous clinics throughout the South West. We are looking for expression of interests for people in the Bunbury and Mandurah regions.
About the Role: The Community Support Worker will play a vital role in providing assistance and support to individuals within the community. This position requires a compassionate and empathetic individual who can effectively communicate and build relationships with the Aboriginal people in our area. The Community Support Worker will collaborate with various stakeholders to ensure the well-being and integration of individuals into the community. The role includes:
Working actively with clients as part of the aged care and community services team. Implementing programs to enhance client skills, independence, and community participation. Encouraging personal and social relationships among clients. Liaising with elderly clients to promote engagement in home support and respite activities. Encourage Elders to follow recommended referral processes for specialists and health checks. Foster collaborative relationships with other health service providers. Certificate III in Individual Support or similar; or willingness to obtain. Demonstrated experience in working in aged and community care services. Sound knowledge of the aged care and community sector, understanding of Aged Care Client Rights, Aged Care Accreditation Standards and Outcomes. Knowledge of health and well-being issues affecting Aboriginal and Torres Strait Islander people with a strong commitment to improving well-being outcomes. Proven ability to work both independently and as part of a team. Ability to work autonomously and display initiative, self-motivation, time management and solution focused skills. Demonstrated interpersonal skills including the proven ability to liaise effectively with and provide high quality client service to a diverse range of people in a professional manner while maintaining confidentiality and discretion. Demonstrated computer literacy with experience using Microsoft Word, Excel, databases (including iCare), email and internet usage. A certificate or tertiary qualification in Community Services, Aged Care or similar field. Current knowledge of legislative obligations for Equal Opportunity, Disability Services, Occupational Safety and Health and how these impact on employment and service delivery. Why Join SWAMS?
