About the Opportunity At Uniting, our Case Managers are called Community Support Partners. This role is responsible for the case management of clients who have complex care needs, making a difference in the delivery of quality and collarborative Home and Community Care services. The Community Support Partner will utilise specialist strategies that are person-centred, to produce outcomes that enable clients to effectively participate in decision making related to their care, access services, and to keep them engaged in their care journey.
This is a permanent Full time, can be based at our office in Tamworth.
About you Tertiary or Diploma qualification in a relevant field, such as Aged Care, Health, Business and/or working towards or equivalent years of experienceCurrent Australian Health Professional Registration Authority (AHPRA) and/or Case Management Society of Australia and NZ certificate, in line with qualification/sExperience in the Aged Care and/or the Health environmentSignificant understanding for client needs, to convert these into positive service delivery outcomesAbility to effectively plan and manage budgetsConfident in building and maintaining strong working relationships with internal and external stakeholdersPassion for social change and contributing to an organisation of influence for vulnerable, at risk and disadvantaged members of the communityProficient in using multiple computer applications, as well as the willingness to learn new systems/programsFirst Aid CertificateCurrent driver's licence and access to a comprehensively insured vehicleBenefits & Culture At Uniting, we welcome and celebrate everyone regardless of lifestyle, ethnicity, faith, sexual orientation and gender identity and proudly create a safe and inclusive environment for staff and clients.
