Community Support Services provide a range of programs to residents of Hume City Council which support the independence of older people and younger people with a disability.
These services include Social Support Programs including recreational-based activities and social outings, Food Services such as delivered meals and Café Based meals, and Community Transport Programs.
What you'll be doing: Provide support to clients, seniors, and residents to access services to improve their health and wellbeing and reduce social isolation.
Assist with the delivery of programs and services which enhance social connection and promote independence and choice.
In partnership provide support in a manner that engages clients in maintaining their own abilities.
Work in collaboration with Team Leaders to ensure the efficient running of the programs and activities.
Contribute feedback from clients and seniors to enhance the planning and development and ongoing delivery of services and activities.
Attend programs, activities, outings, and staff meetings as rostered.
Work flexibly across the three service delivery areas including delivered meals, social support, and community transport.
The skills and experience you'll bring to the role: Certificate III Community Services, Certificate IV Disability, or equivalent or knowledge and skills gained through relevant on-the-job experience.
Experience in the delivery of Commonwealth Home Support (CHSP) and Home and Community Care Program for Younger People (HACCPYP) recreational, food services and transport programs for older people, adults, and younger people with disabilities in a community setting.
Intermediate Microsoft Office suite skills Good oral communication skills and ability to communicate with a wide variety of groups and individuals.
Ability to drive Council's fleet of accessible buses, as required for programs First Aid Certificate.
Food Handling Certificate Current driver's licence.
