Community Support Coordinator, Regional NT Community Support Coordinator - Regional NT (Part-Time) Join HeartKids in making a difference! We are looking for two part-time Community Support Coordinators (2-3 days per week) to support families impacted by childhood-onset heart disease (CoHD) across the Northern Territory.
About the Role HeartKids is a national not-for-profit dedicated to supporting individuals and families impacted by CoHD, including Rheumatic Heart Disease (RHD), which disproportionately affects Aboriginal and Torres Strait Islander communities in the NT. We encourage applicants with lived experience of CoHD or RHD, or a background in community support, especially Aboriginal and Torres Strait Islanders.
This hybrid role is based in Darwin and at the Royal Darwin Hospital's pediatric cardiology clinic, with occasional remote work. Salary packaging is available.
As a key member of our support team, you will:
Provide direct support to families impacted by CoHD via in-person and digital platforms Connect families with HeartKids programs, peer mentors, and advocacy networks Facilitate coordination with health providers and social workers Represent HeartKids at community events and run local support activities Occasionally travel to Katherine and Alice Springs to extend support Key Requirements: Lived experience of CoHD and/or Cert IV in Community Service Open driver's license, Working with Children Clearance, Police Clearance, and required immunizations Strong empathy, adaptability, and communication skills Self-motivated, able to work independently, and committed to delivering exceptional support Your application will include the following questions:
