About the role: Community Nurses play a pivotal role within our Nurse-led model, fostering collaboration with a variety of community-based services.

In this role, our Registered Nurses are entrusted with delivering person-centred, evidence-based nursing care that drives positive health outcomes.
You will assess, plan, implement, and evaluate individualised care in close collaboration with patients, their families, and a multidisciplinary healthcare team.
Building therapeutic and professional relationships with individuals, families, and the broader community, you will provide compassionate care that meets the unique needs of each patient.

Where you'll be working: Community and Aged Care Services Greater Newcastle Sector Community Nursing are currently seeking Registered Nurses for the creation of an eligibility list for existing and upcoming Permanent and Temporary Full-time and Part-time positions at any of our six locations.Community nursing provides clinical services to patients either face to face in their own home, within our dedicated clinics and via virtual care.

Current registration as a Registered Nurse with AHPRA.About you: You are a highly motivated and passionate Registered Nurse, bringing with you a wealth of clinical experience in medical, surgical, rehabilitation, and aged care.
Your versatile skill set seamlessly transitions into community-based models of care, where your expertise and dedication to clinical excellence shine in delivering outstanding patient-centred care.You possess a strong ability to advocate for and deliver individualised, person-centred care, ensuring that each patient's unique needs and preferences are at the heart of every clinical decision.You are a proactive problem solver with exceptional communication skills, adept at engaging with clients to deeply understand their needs and elevate the quality of service delivery.You possess the capacity to engage with others in both a personable and highly professional manner, fostering meaningful connections while maintaining a commitment to excellence.Additional information: If you are currently employed with HNE Health in a position of the same grade, you may not need to submit an online application to be considered.
