As a Community Nurse in the Assertive Outreach Van program, you will engage with clients in the western suburbs of Melbourne to address health and psychosocial needs related to substance use. Your role will involve assertive outreach, harm reduction education, and facilitating connections to health, housing, and social services. You will play a key role in providing flexible, trauma-informed care to clients with complex needs, both on-site and through street outreach.
Key Duties: Conduct assertive street outreach as part of a multidisciplinary team.Provide leadership and coordination to outreach teams.Offer harm reduction services, including needle exchange, overdose prevention, and health education.Support clients in accessing community services such as housing, pharmacotherapy, and social inclusion programs.Assist in community engagement activities aimed at reducing stigma and promoting safety.Deliver health promotion programs and participate in community-based research projects.Skills and Experience: Tertiary qualifications in Nursing and registration with AHPRA.Experience in community nursing, particularly with clients experiencing homelessness or substance use issues.Knowledge of harm reduction principles and recovery-oriented care.Ability to work independently and within a multidisciplinary team.Strong communication skills, with the ability to navigate challenging situations calmly and assertively.Familiarity with providing care to culturally and linguistically diverse populations and marginalised communities.A valid Working with Children's Check, Police Check, and Victorian Driver's License are required.Benefits: Full-time, fixed-term position with flexible work arrangements.Salary packaging opportunities.Professional development and training programs.Work within a collaborative team committed to community health and well-being.
