Community Nurse - Bunbury

Belong at Home (CHC) is seeking enthusiastic, motivated and caring people to join our South West team as a Community Nurse as an Enrolled or Registered Nurse.
We have permanent part time positions available for a Community Nurse with a highly respected not-for-profit community organisation providing care and support services for our Bunbury areas.
As a Community Nurse you will: Undertake clinical assessments of home care clients (RNs)Provide excellent care and service to clients in line with their personalised care plansMonitor chronic diseases and palliative conditionsPerform wound care, continence care, palliative care, medication management and management of chronic diseasesAchieve the optimum care for clients by being responsive to their changing needsLiaise with and refer to other organisations as required to provide for the clients' needsAssist the clinical leads with other duties as requiredHold current AHPRA registrationBe available to work (M-F) - school hours availableAged care experience is an advantageHave excellent verbal and written communication skillsShow genuine enthusiasm, care and compassion in your commitment to working with the elderlyProvide a National Police ClearanceHave Influenza Vaccinations (exemptions considered)A competitive remuneration package including salary packaging up to $15,900 per annum to reduce the amount of tax you pay.
