About the role The Community Mental Health and Wellbeing Service (CMHWS) is a network of hubs designed to meet the increased need for immediate mental health. The hubs act as a 'front door' service to the mental health system where people can access services without eligibility criteria or referrals. CMHWS supports people of all ages experiencing psychological distress or mental ill health and offers a blend of face-to-face support in the community, telehealth and online services. The service delivers wellbeing supports tailored to a client's individual needs including early intervention, specialist referrals, clinical care and treatment, and support for families and carers.

We are seeking a compassionate, self-motivated Community Mental Health Practitioner to work in the Western suburbs area. Working from home, you will deliver positive, person-centred recovery-orientated supports to clients to achieve their recovery goals, promote wellbeing and live safely within the community. As this position requires travel across the metropolitan area, a fleet car or allowance is provided.

Key Responsibilities: Deliver high quality in-person and outreach services and facilitate positive educational and social experiences. Support clients to remain in the community, promote their wellbeing and enjoy a meaningful life through a staged approach to recovery. Provide information and mentoring to clients to establish relationships with families and carers, create community networks and access relevant services. Plan and deliver group programs that aid clients to build their skills and focus on recovery. Collaborate with a range of services and the community to ensure a coordinated, integrated response to clients' recovery goals. What you'll bring: Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related fields. Expertise in working with adults and young people with mental health issues, complex needs and interacting with families and carers. Proven capability to implement therapeutic, recovery-oriented frameworks and practices including risk mitigation, trauma-informed care and family-inclusive practice. Relevant experience in facilitating group activities for clients experiencing mental ill health. Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
