About the Role: The Home Support Transition Coordinator works collaboratively with our Community Care Managers and Welcome Team to ensure customers preparing for a Home Care Package are supported by co-designing a goal-orientated support plan, based on the recommended services set by the Regional Assessment Service (MAC). A rewarding full time role based in Mile End. What you'll do: Coordinate services for your allocated customer load. Meet individual KPI's and contribute to team targets. Conduct reviews, develop support plans, refer for clinical and care interventions, and review customer progress. Liaise effectively and positively with external stakeholders such as RAS, MAC and ACAT. Undertake risk assessments of activities and services for individual customers. About You! You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.  Knowledge of 'Consumer Directed Care'. Ability to manage budgets. Ability to assist individual requirements, including customers with a range of complex needs. A flexible approach to your work with the ability to prioritise and multi-task. Empathy, understanding and a high degree of integrity and diplomacy. Excellent interpersonal, communication, negotiation, and problem-solving skills. You will need: Certificate IV in Community Services/Aged Care/Service Coordination or relevant Health Professional Qualification or equivalent experience. Demonstrated experience in assessment, planning, provision and evaluation of services. Intermediate/advanced level knowledge of Microsoft Office. Excellent documentation and administration skills. Enthusiastic team member and problem solver. To care about supporting older people to live a Good Life. The successful candidate will need to provide an up-to-date COVID vaccination statement (at least 3 doses) and a satisfactory police clearance for working with vulnerable groups, less than 12 months old before commencement.
