Position Vacant - Community Home Support Co-Ordinator
Position Type - Part-time – 15hrs per week
Primary Responsibility: To co-ordinate the delivery of Home and Community Care (Community Home Support Program) services to maintain Aboriginal frail and aged, their spouses and extended family members who are at risk of premature institutionalisation to allow them to remain in their own homes.
