The Community Engagement Coordinator – Aged Care Assist (ACA) sits within the Population Health Team and will lead the Aged Care Assist engagement team by conducting on the ground community engagement activities and initiatives, while overseeing two (2) Engagement Officers.
This position will lead, develop, and implement community partnerships and engagement initiatives, platforms and processes that align with DPV Health's strategic vision. The Community Engagement Coordinator will be responsible for building the Aged Care Assist Team's capability and capacity to meaningfully engage and evaluate the work with communities, consumers, and stakeholders. The primary aim is to increase referrals to the in-house Care Finder team, who will support the clients to register for a Home Care Package via My Aged Care.
The broader scope of the role is to successfully engage and achieve community outcomes with a diverse range of community partners, within DPV Health, across local governments, current consumers, and the wider community across DPV Health's Catchment (Hume, Whittlesea & Mitchell Shire LGA's). The role has a focus on aged care sector and consumer engagement but also encompasses a range of functions across the breadth of DPV Health services, from strategic policy development through to on-ground program delivery support.
Desired Skills and Experience Bring your great, everyday! What you'll bring to the role:
Tertiary qualification to a degree level in Community Engagement/Development, Health Promotion, Public Health, social sciences, or other relevant fields. Strong knowledge of the aged care sector and the My Aged Care system, the eligibility and steps required for community members to register and apply for subsidised supports in their community. Ability to provide information and assisted referral support for community members to access mainstream health support services outside of My Aged Care supports. Strong knowledge in community engagement, community development and co-design principes. Experience working with diverse and intersecting communities. Experience and skills in developing, establishing and sustaining interpersonal relationships. Open minded attitude with an interest in innovation and challenging the status quo. Passion and persistence to change community behaviours, opinions and attitudes. Ideally have experience within the healthcare industry and an understanding of the current principles and practice supporting community health services and funding. The fine print Successful candidates will be required to undergo a National Police Check, hold a valid WWCC and NDIS Worker Screening Check. Full driver's license and travel between sites is required. Outside hours and weekend work is occasionally required.
