About Us

ADA Australia is a not-for-profit, independent, community-based advocacy and education service with 30 years' experience in supporting and improving the wellbeing of older people and people with disability. About the Role

The Community Connector provides specialist and intensive assistance to help older people within the target population to understand and access aged care and connect with other relevant supports in the community. This involves utilising an assertive outreach approach to engage and build rapport with potential clients as well as local intermediaries, stakeholders and networks. Key Duties

Provide information, support, and representation to help clients understand and access aged care services, including My Aged Care. Engage with relevant community supports and stakeholders to assist clients in accessing services. Proactive Outreach: Identify and engage with potential clients and stakeholders within the target population; develop effective ways to connect and build relationships with potential clients. Conduct regular check-ins and follow-up with clients after services begin. Use a comprehensive approach to address clients' psychological, emotional, and physical needs, and remove barriers to aged care access. Networking: Build partnerships with other organizations to promote ADA Link services; promote ADA Australia at events, forums, and networking meetings. Maintain high standards of documentation and consultation, ensuring adherence to all statutory requirements. Identify and report service gaps and needs to the manager. Develop and meet targets outlined in the annual work plan. Liaise with other care finder services and support groups for older people. Forge strategic partnerships with diverse cultural and community groups, including Indigenous and LGBTIQ+ communities.

About You

Experience working with older people or disadvantaged/vulnerable groups and operating within a social justice framework. A tertiary qualification in human services /social work / behavioural science / community services or equivalent combination of relevant experience, education and/or training. Demonstrated ability to understand and apply legislation, policies and procedures, including the ability to identify issues and problem solve. Excellent written and verbal communication and presentation skills. High level computer skills – Microsoft Office Suite or equivalent. Commitment to continuous quality improvement and to delivering high quality service.

Essential Requirements

Be based in Roma or Charleville QLD. Valid Australian Driver's license + safe and reliable vehicle. Current (less than three (3) years old) pre-employment National Criminal History Certificate. Valid NDIS Worker Screening Clearance. Valid Working with Children/Blue Card.
