Traralgon, West Gippsland & Latrobe Valley VIC Aged & Disability Support (Community Services & Development) The Filipino Community Council of Victoria, Inc. (FCCVI) is a duly incorporated non-political, non-stock, non-profit, and benevolent community service organisation, founded in 1988. FCCVI is recognised as the umbrella body of the Filipino organisations in Victoria. Its existence came about as a direct request from the Australian Government in the late 1980s through the then Minister of the Department of Immigration to be the primary contact in communicating and collaborating with individual Filipino organisations rapidly increasing at that time. We are seeking suitably qualified Community Care Workers to work in Gippsland, Moe, Newborough and Traralgon to assist in maintaining and enhancing the physical, emotional and social wellbeing and independence of older people and people with disabilities who are assessed as needing support to remain in their own homes. Support and assist clients with tasks of daily living as specified in their Care Plan:

Home Care: assisting clients with domestic responsibilities such as laundry, ironing, changing bed linen, meal preparation, tidying up, vacuuming, dusting, cleaning bathroom and toilet. Personal care: assisting clients with showering/bathing, toileting, dressing, grooming, and assistance with mobility, prompting of medication. Community Access: assisting clients with shopping, paying accounts, banking, monitoring clients' safety and well-being, and any other areas of concern to the Case Manager/Coordinator. Support Service: in-home respite for carers, transport service to medical appointments and other activities as requested, companionship, group activities.

Administration:

Monitor and report on client's conditions as required. Provide information and feedback to the Case Manager/Coordinator and other health professionals as required. Liaise with Case Manager/Coordinator for additional service, support and resources as required. Complete required client records and documentation. Carry out other tasks as assigned by the Case Manager/Coordinator.

KEY SELECTION CRITERIA:

Certificate III in Aged Care or Home and Community Care, or the willingness to undertake studies to achieve this minimum. Level 2 First Aid certificates or current Cardiopulmonary Resuscitation (CPR) certificate or the willingness to undertake studies to achieve this minimum. Commitment to supporting elderly people in their home to maintain their own lifestyle and an understanding of the importance of person-centered care. Demonstrated ability to provide social support to HCP clients and report changes to supervisors. Good communication skills, ability to speak fluently in Filipino (Tagalog) and English for non-Filipino clients - knowledge of another local language/dialect is an advantage. Understand privacy and confidentiality issues, whether related to the client or the client's family. Good understanding of professional boundaries and work ethics. Courteous, supportive, patient, and non-judgmental. Accept direction and use initiative where appropriate. Knowledge of OH&S. Driver's licence and access to a comprehensively insured vehicle.
