Classification: SCHADS Level 4 with a salary range of $85,205 - $91,686; per annum + Superannuation

About your new role
Community Care Coordinators will provide one-on-one, flexible, psychosocial support and care coordination for up to three months. People will be supported to develop and carry out wellbeing plans. Support may include working alongside consumers and families to navigate systems and services, and connection with a range of supports available to them. Community Care Coordinators will build and maintain relationships with a number of stakeholders and community partners to facilitate holistic care for people accessing the service.
Community Care Coordinators will collaborate closely with the Service Manager and the Healing and Recovery team at Danila Dilba Health Service. Community Care Coordinators at Neami National function within a collaborative team approach and are supported by regular opportunities to reflect upon their practice and access comprehensive training and development.
What you will bring to the role
Significant experience and understanding of working with people experiencing distress, with a mental illness, suicidality and/or deliberate self-harm
Sound understanding of the personal, family, and social factors associated with suicidal ideation and attempts
Demonstrated interest/and or experience working with cultural and community groups known to be at higher risk of suicide (e.g. males, Aboriginal and Torres Strait Islander peoples, LGBTQIA+ community, people who identify as CALD, youth)
Experience liaising with external service providers, including specialist mental health services, GP's and other community-based services and organisations.
Demonstrated ability to work with partner agencies and organisations to strengthen community-based prevention and intervention initiatives

What the role requires
National Criminal History Check (police check) - disclosable outcomes considered
Current Working with Children Check - required before commencement of work
Current Australian drivers licence and access to a car
