Service Delivery Coordinator - Support at Home Community Based Support (CBS) is a not-for-profit Tasmanian organisation delivering aged care and disability services to enable people to live safely in their homes and enjoy access to the community as they wish.
Our work ensures quality of life is enhanced by providing a range of support services that are tailored to meet individual needs and goals to maximise independence.

Service Delivery Coordinator – Support at Home

Our Service Delivery Coordinators work within a team environment to provide quality person-centred support to a caseload of Aged Care clients.
In addition, the Coordinator will utilise their leadership experience to effectively supervise the Field Workers within their regional teams.

You flourish in an environment where you can multi-task and engage with a diverse range of community members, advocating for individual's needs You reflect our values of Quality, Respect, Collaboration and Accountability You bring minimum 2 years' experience within a similar position in a large multi-faceted organisation (desirable) Your experience reflects your knowledge and understanding of various disabilities, working with individuals who live with disability and their carers/support networks What you must have:
Relevant qualifications and experience in the Community Services sector (preferably including Aged Care services) Current Working with Vulnerable People Registration (or ability to obtain) A National Police Check that is no older than 3 months (or ability to obtain) To apply:
