Community Aged Care Manager CHSP ProgrammeAbout First Peoples' Health and WellbeingFirst Peoples' Health and Wellbeing (FPHW) is a dynamic Aboriginal Community Controlled Health Service aiming to improve access to affordable primary health care in urban Melbourne. FPHW has two clinics, one based in Thomastown and the other based in Frankston, both offering fully bulk billed primary healthcare that is culturally safe and trauma informed for the Aboriginal and Torres Strait Islander community and their families.From November 2024, First Peoples' Health and Wellbeing will commence services under the Commonwealth Home Support Programme (CHSP) to assist eligible Aboriginal and/or Torres Strait Islander peoples to remain living at home and in their community for longer. The delivery of the services will be achieved through culturally safe, strength-based entry-level home supports, tailored to individuals' needs in the areas of:NursingPersonal CareSocial SupportMealsTransportHome MaintenanceHome ModificationsServices will be delivered by a multidisciplinary team of healthcare, social, and allied health professionals experienced in delivering strengths-based and trauma-informed programs and services.About the roleThe Community Aged Care Manager will act as the role model, point of contact, and be responsible for overseeing the establishment, management, and operations of the Commonwealth Home Support Programme. Responsibilities include ensuring high standards of client care and appropriate risk management and documentation while maintaining strong community relationships. This role requires a sound understanding of My Aged Care Australia, Commonwealth Home Support Programme (CHSP), and the Aged Quality and Safety Standards.ResponsibilitiesEstablish and operationalise the Commonwealth Home Support Programme (CHSP) using a strength-based approach that is community-led.Lead accreditation requirements as per the Aged Care and Safety Commission standards to operate CHSP services.Develop and coordinate accessible referral pathways for eligible community members to obtain a Regional Assessment Service (RAS).Identify and recruit internal workforce and establish external stakeholder partnerships to support the services being delivered efficiently and effectively.Manage local partnerships in consultation with the Senior Leadership Team (SLT) and represent the service at external meetings as required.Coordinate reporting through the My Aged Care portal (DEX) according to the CHSP service agreement.Manage people employed within CHSP, including time sheets, leave requests, professional development, and performance reviews.Coordinate and chair CHSP team meetings (monthly) and contribute to the Clinical Governance Committee and Quality Committee as required.Manage complaints/compliments/feedback and maintain all registers (risk, complaints, clinical incidents, etc.).Ensure documentation and data are confidentially maintained on the service's software systems.Responsible for the oversight of work health and safety and risk management as per requirements of Aged Care and Safety Commission Standards.Drive the culture of the program to ensure a harmonious, trauma-informed, and culturally safe workplace for staff and clients in line with FPHW's values.About youYou will be a friendly, respectful person with strong organisational skills, ability to multi-task, and prioritise. You will have a sound understanding of barriers identified entering the aged care system and be an advocate for First Nations people, to maximise independence at home and in the community by enhancing overall health, wellbeing, and quality of life.
