LME Care, a premier disability support provider in Brisbane, is looking for a dedicated and dynamic Community Access Coordinator to join our team. As a Community Access Coordinator, you will play a crucial role in enhancing community participation and fostering independence for our clients. You will work collaboratively with individuals to identify their unique goals, develop access plans, and facilitate connections to community resources and activities that promote inclusion and engagement.You will also be developing and expanding Community Access at LME, ensuring a top-notch Community Access experience by creating opportunities, building networks, and developing person-centered community programs.
ResponsibilitiesAssist clients in identifying their interests and goals related to community accessDevelop individualized community access plans based on assessments and client needsBuild and expand the Community Access section of LME CareCoordinate and facilitate participation in community activities, programs, and eventsCollaborate with families, support teams, and external agencies to optimize service deliveryMonitor and evaluate the effectiveness of community access programsProvide guidance and support to clients in overcoming barriers to participationEnsure compliance with all relevant policies and procedures, including health and safety regulationsMaintain accurate documentation and reporting on client progress and program outcomesEngage in ongoing professional development to enhance skills and knowledge in community accessMinimum RequirementsExperience in disability, community development, internal/external relations or related fieldsCertification in Support Work, Community Services or related field highly desiredConfident in developing and maintaining positive relationships with clients, families, and community organizationsStrong understanding of the challenges faced by individuals with disabilities and their familiesExcellent organizational, communication, and interpersonal skillsAbility to market events, programs and community service opportunitiesDemonstrated ability to identify available services and service gapsProficiency in creating and implementing individualized plansProblem-solving skills and the ability to navigate complex situationsCurrent first aid and CPR certificationValid driver's license and access to a reliable vehicleUnderstanding of the NDIS framework and community resources available to individuals with disabilitiesAvailability to work flexible hours including evenings and weekends may be requiredRemunerationSCHADS Level 4.1Access to salary sacrificing benefits
