What you'll be doing The Wellbeing and Health in-reach Nurse (WHIN) Coordinator CNS2 is responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools. The WHIN Coordinator CNS2 is a Registered Nurse who works in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex health care coordination for school aged children and young people in these schools. The WHIN Coordinator CNS2 role requires leadership, specialist skills, capacity for a significant level of autonomy and independent work in an interagency context. Coordination of access to services across multiple agencies, professions and groups will be part of the role. The Scope of the WHIN Coordinator role is to: Work with local school principals, school wellbeing teams, school counsellors and psychologists, school staff and local services to deliver the WHIN Coordinator model of careBuild trusting relationships with students and families to identify health needs and address barriers to accessing servicesEstablish a family focused approach to connecting students and families to servicesIdentify and triage the health needs of students and familiesAssist in the coordination of appropriate early intervention, assessments, and referral to services and programsContribute to care coordination and case management of vulnerable children, young people and familiesSupport access to health promotion programs and services to improve the health and wellbeing of the school community and provide support for students and families to improve health literacy and access to servicesWork within locally agreed protocols and governance arrangements across Health and Education sectorsBuild and maintain relationships with service providers and establish effective referral pathwaysCollect and report on service events as required, maintain confidentiality and accurate health care documentationActivities out of scope are: Providing lessons on health and wellbeing within the classroomUndertaking individual or group mental health interventions with studentsProviding population screening for health and development (although the WHIN Coordinator may coordinate access to screening services if required)Providing support for students who require health care procedures while at school, including toileting, tube feeding, tracheotomy suctioning, diabetes management Current registration with the Nursing and Midwifery Board of Australia with relevant post -registration qualifications and at least 3 years experience working with children and young people, a post registration qualification related to the specialty area of child and family health, or paediatrics, or adolescent / youth health nursing practice or other related area.Demonstrated understanding of and experience working with children and young people, including marginalised and at-risk groups, and health promotion, prevention and early intervention approaches and evidence based practice.Demonstrated capacity to engage with families to address various health and mental health problems for children and young people and effectively liaise with health and social services involved in their care.Demonstrated ability to work within an interagency context, with an emphasis on relationship building, service delivery, quality of care and team cohesion, and an ability to identify and improve opportunities to enhance integration of care for children, young people and families.Demonstrated excellent interpersonal and communication skills, including advanced verbal and written skills; an ability to negotiate and problem solve; and the capacity to collaborate with and influence health and other professionals.Demonstrated knowledge of and ability to utilise electronic communication and medical record systems across a variety of applications, including eHealth technologies.Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Ministry of Health Policies and National and State Standards in relation to children and young people.Current NSW driver's licence and ability and willingness to travel for work purpose Need more information? 1) Click here for the Position Description 2) Find out more about applying for this position For role related queries or questions contact Grainne Ducat on
