Location – Primarily work from home role with office days required on occasion for training or meetings
Flexible hours and days – Monday to Friday, Full time or part time; no shift work!
Pay rate – $56.73 per hour + superannuation
7 additional leave days per year

We're thrilled to announce that our Aged Care Assessment Services ('ACAS') team is expanding, creating exciting opportunities! ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We have an exciting opportunity for a Clinical Triage Delegate to join the team.
What will your role look like?Your role will assist older people in navigating the assessment process for aged care services by conducting thorough triage evaluations to identify the appropriate assessment pathway and prioritize their needs.
Day to day this includes:Establishing rapport to create a safe and comfortable environment for the client to express their concerns and to confirm screening information utilising the Integrated Assessment Tool (IAT) ToolDetermining eligibility and assessment priority and pathwayConducting risk assessments based on client vulnerabilityGathering information relevant to client circumstances and confirming the information within the referralMaintaining currency of knowledge relating to the Aged Care Act and relevant Aged Care frameworks and standardsTo be successful in this role you'll need:Excellent Communication and Customer Service skills to both internal and external stakeholdersExcellent planning, organising and problem-solving skillsEffective negotiation, advocacy and conflict resolution skillsTertiary qualifications – in a health-related discipline directly related to health, aged care or related specialist area (e.g., registered nurses, medical officers, occupational therapists, physiotherapists, and social workers)Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association.Completion of training requirements as specified under the My Aged Care Workforce Learning Strategy 2023.At least one year's aged care assessment experience preferredWillingness to apply for a Police CheckWhat we can offer you:Join a highly engaged dynamic team where you'll be supported to grow and take on new challengesMake a real difference in the lives of Older Australians – play a critical role in assessing their needs and ensuring they get the right careBuild valuable skills and sharpen your expertise to help you grow a career in aged care servicesFlexibility and fulfillment where you can prioritise work/life balance, build strong relationships with your colleagues and make an impact on clients and their familiesAged Care Assessment Services (ACAS) operate independently of Catholic Healthcare and play a pivotal role in ensuring that Older Australians are assessed for the care and services they truly need.
