Clinical Team Leader | Full Time | Regis Sandgate Musgrave The role of the Clinical Care Team Leader is to provide and coordinate resident care, supervise care staff to ensure an effective, efficient and functioning team under the direct supervision of the Clinical Care Manager and General Manager. As a Senior RN, you will be required to lead and manage the clinical and allied health services under the direction of Clinical Care Manager. As a member of the clinical team, you will perform in accordance with legislation and practice within a professional and ethical framework.
The Clinical Care Team Leader is responsible for coordinating the assessment and evaluation of individual resident care within the scope of practice; to act as a resource for staff; perform competency assessments and have input into training of staff. The Clinical Care Team Leader will liaise with the Clinical Care Manager to ensure resident care outcomes are met.
Duties & Responsibilities Ensure that the care and service is coordinated in accordance with the philosophy, policies and procedures of Regis Aged Care in line with scope of practice and other governing regulations.Lead and participate in the delivery of a high standard of person-centered resident care which includes:Prioritizing the social, emotional, spiritual and recreational needs of all the residents and supporting care staff in working towards these needs.Provide a high standard of ongoing care management and perform nursing interventions in accordance with recognized standards of practice relevant to a Registered Nurse.Coordinate the comprehensive completion of individual holistic assessments and care plans for new and existing residents and evaluate care in line with Regis policy and within scope of practice.Ensure that the coordination of all clinical documentation is completed accurately and in a timely manner in accordance with Regis policies and procedures. These include but are not limited to:Care plans updated and verified, ACFI commenced as scheduled,Clinical assessments are reviewed, updated and validated by an RN,Resident of the day program and weekly progress notes are completed according to home's schedule.3 monthly care plan reviews are conducted as per the schedule.About you As a collaborative and senior member of the operations leadership team, you will possess:
Relevant tertiary qualifications or Australian recognized equivalency.Current registration with AHPRA as a registered nurse.Previous senior clinical care experience in the health sector with clinical experience in aged care.Demonstrated leadership and influencing skills.Sound understanding of care assessment in the Aged Care Sector and use of AN-ACC.Demonstrated effective interpersonal and communication skills (written and verbal).Demonstrated proficiency in the use of a wide range of IT applications.To work in residential aged care, you will need to provide satisfactory background checks, supply an NDIS worker screening clearance, COVID and flu vaccinations, and certification proof.
