The Care Planner is responsible for assessment, care planning, and care evaluation in accordance with company and resident policies and works closely with the Care Manager and Care Team to achieve their objectives.What You Will DoComplete interim assessment and care planning on day of admissionsComplete quality care evaluationsEnsure care plans are developed in true partnership with residentsLiaise with the company revenue and classification team (ACF1/AC ACC) to ensure accurate classification and optimise recurrent fundingProvision of safe, high-quality care in accordance with the Aged Care Quality StandardsPositive resident experience through resident-first approachWho are you and what will you bring?Registration with the APHRA as a Registered Nurse is essentialMinimum 2 years' experience in a care management role in the residential aged care settingDemonstrate strong verbal and written communication skillsHold and maintain NDIS Worker Checks as per NDIS Act 2013 (or willing to obtain)Be passionate about delivering a person-centred approach to careCome experience the charm, history, and warmth of Yackandandah —it's more than a place; it's an invitation to create your own remarkable story!Interested?
