What you'll be doingThe Palliative and Supportive Care Clinical Nurse Specialist 2 (CNS 2) provides expert, compassionate care to achieve quality outcomes.
The role involves assessment, planning, implementation, and evaluation of nursing care in collaboration with the healthcare team.

Essential Criteria: Qualification: Post Graduate qualification in relevant specialty Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA) Selection Criteria: Registered Nurse/Midwife appointed to a position classified as such with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification.
Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning Demonstrated ability to provide leadership in the provision and development of clinical practice through the use of evidence based research, demonstrated understanding of current NSW Policies, National and Standards related to Palliative Care Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based services Demonstrated ability to assess and deliver effective care to patients /carers within a complex clinical environment.
