The Palliative Care Clinical Nurse Specialist is an experienced registered nurse with advanced knowledge of the health needs of patients diagnosed with life-limiting illnesses. They will demonstrate the ability to act as a professional role model and clinical leader, with a high-level capacity for patient advocacy, while working independently and as part of the multidisciplinary team. The successful applicant will be expected to be able to provide evidence-based, clinically sound and holistic patient care in partnership with clients and their families and other health professionals. Part of the role will also include assisting the inpatient Clinical Nurse Navigator to triage referrals, manage waitlists and lead multidisciplinary meetings, while collaborating with internal and external key stakeholders.Our Culture: St Vincent's is committed to employing a workforce that reflects the community we care for.
