As a Clinical Nurse Specialist (CNS) in the Aged Care Assessment Team, you will serve as a professional role model and advocate for clients, showcasing your ability to enact change and provide effective clinical leadership.
In this autonomous role, you will collaborate within a multidisciplinary team to deliver comprehensive assessments and coordinate care for clients seeking access to Australian Government-funded aged care services.
You will support and supervise less experienced assessors, ensuring adherence to the Nursing and Midwifery Board Australia's Standards of Practice while fostering professional development.
Your key responsibilities will include conducting thorough assessments using My Aged Care, providing referrals to Commonwealth-funded services, and offering vital information to clients and their carers.
