The vision for nursing and midwifery at The Sutherland Hospital is to build a united team that provides compassionate care to our community through transformational, supportive and holistic nursing and midwifery.Your new role: The Clinical Nurse Specialist is a Registered Nurse who has responsibility for the coordination and clinical support of emergency department based aged care services and rapid response aged care community services within the division of Southcare (Aged and Extended Care). Duties include efficient and effective management of older people who present to the Emergency Department and reside in the community.Where you'll be working: ASET provides a multidisciplinary consult service for patients 70 years and older who present to the Sutherland Hospital Emergency Department. ASET works collaboratively with Emergency Department staff to achieve optimal care and management of the elderly patient. ASET assesses and identifies complex issues, advocates for patients and their carers, and refers and facilitates access to other relevant services if medically cleared from the Emergency Department.Selection Criteria:Current registration with the Nursing & Midwifery Board of Australia (AHPRA)Relevant post-registration qualification and at least 3 years' experience working in the Aged Care clinical area, or such other qualifications or equivalent experience deemed appropriateDemonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plansDemonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situationsDemonstrated skills in the development & facilitation of patient and carer education (and staff as required)Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with othersDetailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovationDemonstrated competence in the use of health-related information technology.Applications Close: 17 November 2024
