(pro rata) RN2C | Multiple Part and Full-Time Ongoing Contracts Positions working Monday to Friday or across a 7 day rotating roster Join our team and contribute to providing high-quality mental health care in the community!Seeking passionate and organised individuals to play a pivotal role in coordinating and delivering outstanding clinical care within our dynamic Specialist Community Mental Health Service.Responsibilities: Deliver comprehensive clinical care coordination: Facilitate communication and collaboration between patients, clinicians, and community stakeholdersDevelop diverse professional portfolios: Gain experience in depot and clozapine clinics, supporting consumer fitness groups, and facilitating therapeutic interventions.Conduct thorough assessments: Utilise your expertise to perform mental health and psychosocial assessments, risk assessments, and develop personalised treatment plans.Support team leadership: Collaborate with the Team Manager to contribute to a cohesive and efficient team environment.Contribute to quality improvement: Engage in clinical auditing, research, and evaluation initiatives to continuously improve patient care outcomes and service delivery.
